If you haven't already read the information on this page, please do so before completing the registration forms. Failure to provide all of the necessary information, regardless of reason, may result in loss of eligibility for the child.

Cheerleaders have additional fees and costs associated with participation.

All participants are required to have a physical examination dated after January 1 of the current year. You must have your doctor fill in this Medical Form completely.

We encourage all participants to keep higher standards scholastically. We do this through our Scholastics program. We require that 2 copies of all 4 quarters of your child’s report card be mailed to the above address by the deadline of July 1st.

Note : If your child has a GPA below 2.0, a Scholastic Eligibility Form will have to be signed and stamped by the principal of your child's school. The form provides a designated area to be marked with the school stamp.

Football participants are placed on teams according to their age and weight. For more information on this see Age & Weight Requirements.

Cheerleaders are placed on teams using the same age criteria as football players however they do not have a weight requirement.

Important : All applicants must include all insurance information requested.

On the bottom of the registration forms there is an area to select your preference of volunteer position. If you do not select a category you will be placed in the area most needed ( typically the snack bar ). For a list of positions see Volunteer Information

If you wish register by mail you may print a copy of the Registration Form (.doc). Once you have completed the printed form, send the following items to the address below:

  • Completed Forms ( Registration, Parental Consent, Medical Form )
  • Wallet Size Photo (No Hats, No Sunglasses, No Polaroids)
  • 2 Copies of Birth Certificate ( Do not send original )
  • Registration Fee
  • Scholastic Eligibility Form ( if applicable )
